Linea 5 – Neuropsicologia e recupero cognitivo

La Linea di ricerca 5 ha come obiettivo lo sviluppo di nuovi metodi e conoscenze per migliorare la valutazione e la riabilitazione neuropsicologica.

L’attività della linea promuove innanzitutto lo sviluppo di strumenti di valutazione neuropsicologica (come test, questionari e batterie) e di trattamenti neuropsicologici (comportamentali o basati su tecnologie innovative, come la telemedicina o neuromodulazione).

Le attività di ricerca della linea hanno anche come obiettivo lo sviluppo di conoscenze sulle basi neurali alle base dei processi cognitivi che sono valutati e riabilitati includendo tecniche di neuroimmagine e neurofisiologiche, con l’obiettivo ultimo di comprendere meglio le potenzialità del paziente e definire le strategie migliori per la sua riabilitazione cognitiva.

Le attività della Linea vengono svolte con il supporto principale dei Laboratori di Neuropsicologia, Neuroimmagini e Neurofisiologia.

Financial Abilities in neurological diseases. Development of a telerehabilitation program: FINAGE

The psychometric test to assess FA is the first tool available in Italian to measure such capacities. The TR program developed will be the only structured program validated for the Italian population for FA improvement in neurological patients who suffer a great loss of independence. In addition to the patient’s quality of life amelioration, enhancing his/her autonomy, TR use provides support for clinical practice, in terms of costs and efficiency. The MRI studies in Aim 3 will contribute to better define neural correlates underlying FA, extending our knowledge from a structural brain basis to which functional networks are involved in improving insufficient FA. Data obtained in these studies will enrich the theoretical knowledge on the construct of FA, and provide further information on the effectiveness of the rehabilitation program on a neural basis. Moreover, the outcome measures from patients undergoing different rehabilitation strategies will also be compared.

The aim of the present project is to establish the incidence of swallowing disorders and their evolution after rehabilitation treatment in anoxic patients. An evaluation scale of pharyngeal stagnation will be validated in Italian (YALE SCALE), the correlation between stagnation and aspiration, and the effectiveness of standard treatment versus biofeedback in patients with dysphagia will be studied.

We will use computational models and machine learning methods to study the relationship between connectivity and cognitive deficits in patients with stroke, evaluating diagnostic and prognostic prediction even when compared to lesional information alone. The computational modeling of human cognition based on neural networks (starting for example from investigations on reading or computational processes) converges in this field of research applied to clinical neuropsychology, to identify improvements in diagnostic and rehabilitative strategies.

The project aims at investigating two specific executive abilities in stroke patients, namely, error monitoring and error awareness. The working hypotheses are that recovery after stroke improves in 1) patients with better error monitoring and error awareness abilities, and 2) in patients that combined an EF training with standard rehabilitation programs, regardless of their specific impairment.

Clinical testing for neglect is focused on assessment of spatial bias in visuomotor tasks, mostly paper-and-pencil tests (e.g., figure copy, line bisection, cancellation). Their sensitivity is highly variable across studies. Our computerized test might become a standard assessment tool for clinical practice because it prevents ompensatory strategies and uncovers subtle or mild forms of neglect. Note that neglect symptoms are often more severe in everyday life, where multi-tasking is ubiquitous, than in paper-and-pencil tests. A further innovative aspect is the attempt to identify neurofunctional markers that reflect the individual differences between patients in everyday life performance and are predictive of the future outcome of functional recovery. Finally, rehabilitation of neurological patients through videogame play is highly innovative and timely given the growing literature showing cognitive enhancement (especially of attention skills) in neurologically intact adults.

In this project the neuroanatomical and functional bases and the cognitive correlates associated with numerical and financial abilities in Parkinson patients and in right and left stroke patients will be investigated. Objectives of the project will be to define the specific pattern of impairment of this capacity specifically for each pathology considered in consideration of the study of cognitive and neuroanatomical correlates – adaptation of the battery to the peculiarities of the considered pathology (nadl-f short version, version for patients with lesion left hemispheric, etc. ..) – development of a rehabilitation package of financial domains compromised results in the evaluation phase.

We plan to analyze the neuropsychological and structural and functional resonance magnetic data of a sample of patients with mild cognitive impairment and an equivalent sample of control subjects. Data collection is envisaged for the evaluation of daily autonomy, with respect to lifestyles, with the request for detailed information regarding the personal and family situation, the use of free time (with attention to the aspects of socialization and activities with background cultural and associational); to drink and to drink; to smoke; to sleep alterations; motor activity and drug therapy. The verbal and linguistic aspects of these patients will be the object of a detailed evaluation, to understand if the production of non-canonical linguistic forms can constitute an early marker of disease decay and progression.

To improve the quality of the treatments offered to patients with aphasia it is necessary to deepen the lexical and syntactic mechanisms within aphasic disorders and to study the architecture of linguistic processing in comprehension and production. This in association with instrumental analyzes such as Magnetic Resonance (RM) and / or Magnetoencephalography (MEG). In addition it is considered necessary to verify the effectiveness of the treatments available in the literature. We aim at analyzing how the recovery of the anomaly occurs and the learning of new words in patients with aphasia, use structural priming as a treatment tool and carry out a systematic review of treatments for dysgraphia.

The aim of the present study is to carry out a systematic investigation on a series of very frequent, degenerative and vascular neurological diseases, We aim at identifying the type of deficit in emotional recognition, also in relation to the severity, the state of progress of the disease and the type of cognitive, sensory and motor symptoms. The emotions codified by social psychology as fundamental emotions (happiness, sadness, anger, fear, disgust and surprise) will be investigated. All recruited patients and controls will undergo 1.5T cognitive and brain MRI scans. The application of the FEAST battery (Face Emotion Action Stimulus Test) will allow to define a specific pattern of impairment of this capacity in each pathology considered and to study the correlations with other cognitive functions and with the brain areas involved through the use of techniques of neuroimaging as VBM and VLSM.
